How to access the bonus content
To access the bonus content, you must fill out the form below proving that you bought a copy of the book. You can do this however you choose. Approved methods include:
- Send an image of the receipt. You must sign and date your receipt to prevent duplicate entries (not that anyone would do that).
- If you ordered an autographed copy of the book from me, use the name and email address you used to make the purchase and I will verify it.
- If you bought a copy at a book fair, send an image of the signed cover page.
- If you bought the book on Amazon, copy and paste the text of your email receipt.
- If all else fails, turn to the cover page, write "Property of YOUR NAME" and sign the date, then send me the image. This will prove the book is yours, since you hopefully wouldn't deface a library book. The name and date will prevent duplicate entries (not that anyone would do that).
- Any other method that proves that you bought the book (subject to my approval).
Once your entry has been approved, you will be sent information on how to access the exclusive extras. If there is a problem with your entry, I will notify you so you can make corrections and submit it again. I have the final say on whether your entry is approved or not, no appeals or negotiations.
